Revised 10/12/2009
24-Hour Cancellation Policy
Connec-to-Talk requires at least 24 hour notice in the event of a cancellation or you will be charged for the session. If exceptional circumstances exist, we may accept a minimum of 12 hours notification. There will be no exceptions to this policy.
Session Length
Based upon clinical observations and/or diagnostic testing, your child’s therapist will recommend an appropriate length of time for a session. Your therapist will follow the pre-determined therapy time unless otherwise specified. Therefore, should you like a progress update of the session please let the therapist know in advance so that she/he may end with enough time to provide an update. Otherwise, you can contact your child’s therapist at a later time to receive an update.
Late Arrival
A credit will not be issued if you arrive late to a session and the session will not be extended due to a late arrival. There will be no exceptions to this policy.
Payment for Services Received
Connec-to-Talk requires payment to be received by the end of every week following services rendered. A statement of payment will be issued at the end of every 4 week period. If payment is missed two or more weeks in a given 4 week period then a late fee of $50 will be added to the monthly statement. No exceptions will be made to this policy unless prior arrangements have been made with a managing partner of Connec-to-Talk. Please place your payments in the white mailbox located in the lobby area.
Observations
Connec-to-Talk welcomes observations from outside professionals. We ask for seven days (7) notice prior to an observation. Please confirm all observations with your child’s therapist before scheduling.
Report Writing Charge
When a therapist writes a report for your child, an hourly rate (not to exceed four (4) hours) will appear on your monthly bill. This report writing rate is determined by dividing the 60 minute session rate of your therapist by fifty percent (50%). No exceptions will be made to this policy.
Team Meetings
When a therapist participates in a team meeting for your child you will billed at the therapist’s hourly treatment rate.
Data and Record Keeping
Children receiving ABA services will require regular maintenance and tracking of data to maintain accurate record keeping. When a therapist participates in maintenance of data for a child’s ABA program you will be billed at the therapist’s half rate. The rate is determined by dividing the 60 minute session rate of your therapist by fifty percent (50%). No exceptions will be made to this policy.
Phone Consultation
When a therapist provides phone consultation, to a parent or related service provider, and the conversation directly relates to a child who is receiving services, a prorated hourly rate will be applied after the first fifteen (15) minutes of the phone call. The rate is determined by dividing the 60 minute session rate of your therapist by fifty percent (50%). No exceptions will be made to this policy.
